Referrals can be faxed to us at 623-288-5074 - Please include the lab order and any patient demographic information.

Just a quick note: Our mobile phlebotomy services are private pay for the convenience of at-home or on-site collections. However, lab testing is still billed to the patient's insurance as usual.
